-Update Your Information in the Alumni Database-
Release and Consent Agreement

1. Knowing you have and will expend substantial expense and time in reliance upon this
release and consent, I irrevocably consent to the use by the Novato Chamber of Commerce,
(NCOC) and those acting with its permission and authority, of my first and last name, my company’s name, graduation year, all contact information and any and all photographs or video images which the Novato Chamber of Commerce has arranged to be taken of me or in which I may be listed in the Novato Leadership Alumni section.

2. I understand the Leadership Alumni database is a public directory and its information is available worldwide.  I confirm that the information appearing in this public directory was supplied to the Novato Chamber of Commerce by me and is correct and up to date.   

3. I have not supplied any information that I do not want released to the public.  I release the Novato Chamber of Commerce of any responsibility for the use of the information I have provided to them for this public directory.  

4. Please contact the Novato Chamber of Commerce at info@novatochamber.com if you would like your information deleted from the Novato Chamber of Commerce Leadership Alumni public directory.

5. I understand that only the Novato Chamber of Commerce is authorized to update and publish this information on the NCOC Leadership Alumin Web site section. It is my responsibility to make sure my information is accurate.

6. This agreement constitutes the sole, complete and exclusive agreement between the Novato
Chamber of Commerce and me. I am not relying on any other representation whether
oral or written.
